Specifics of physiotherapy in patients with bronchial asthma

The bachelor thesis deals with the possibilities of physiotherapy in patients with asthma bronchiale. The aim of the theoretical part of the thesis is to describe the problem of bronchial asthma and to describe the possibilities of physiotherapy for patients suffering from asthma. The theoretical part of the thesis includes a comprehensive description of the disease, specifically definition, prevalence, etiopathogenesis, clinical symptoms, diagnosis, classification of the disease and treatment options. The anatomy of the respiratory system, respiratory muscles, a description of physiological breathing and its pathology in asthmatics is also included. A relatively large part of the theoretical part consists of physiotherapy options. Asthma bronchiale is a chronic inflammatory disease of the respiratory tract manifested by symptoms of bronchial obstruction such as dyspnoea, cough, wheezing phenomena when breathing and chest pressure. The disease can be accompanied by asthma attacks of varying frequency and severity, which can be life-threatening. The cause of these difficulties is the presence of chronic inflammation in the bronchi together with their hyperreactivity. The prevalence of asthma continues to rise worldwide, along with the increasing prevalence of allergic diseases, which may be influenced by the worsening air quality. Asthma bronchiale is an incurable disease that affects people of all ages. With timely diagnosis and the use of pharmacological treatment, the symptoms of the disease can be eliminated and thus the disease can be controlled. Diseases of the respiratory system also have a negative effect on the musculoskeletal system, so physiotherapy has its place in the comprehensive treatment of asthma. Physiotherapy treatment focuses mainly on restoring the correct breathing pattern, correcting body posture, eliminating muscle imbalances and improving physical fitness. The aim of the practical part of the thesis is to propose physiotherapy procedures suitable for patients with bronchial asthma and to evaluate their effect. The form of qualitative research has been chosen for the practical part. The research includes 3 case studies of adult patients with asthma. Influence of climatological spa care on bronchial asthma in children and adolescent

In the world and in the Czech Republic, climatotherapy is a recognised tool in the complex rehabilitation care of patients with bronchial asthma. Its use is suitable as a complementary therapy that serves to improve overall health. This diploma thesis provides a theoretical insight into the subject. Furthermore, this thesis contains graphical results of the probands' entrance and exit examinations, which help to clarify the effectiveness of climatological rehabilitation treatment. In the conclusion of the diploma thesis, the results are evaluated and discussed with other authors. The established research questions have been confirmed, both by the results of the examinations and by using professional sources.
Nursing care about adolescent with asthma bronchiale

My thesis is focused on adolescents with bronchial asthma. I described bronchial asthma, pathophysiology, treatment, regimens, nurse education and the specifics of communication with adolescents. First goal was to find out how adolescents abide by regime, whether they have encountered alternative methods of treating asthma, and whether there are gender differences. Questioning method has been used, where I created the online questionnaire and placed it on the social network Facebook. Second goal was to map the role of a nurse in an allergology clinic. Online questionnaire method has been used via online Skype communication. The results show that there are only minimal, even negligible differences in compliance with the regime's measures in relation to gender. Furthermore, it has been shown that, thanks to medication and appropriately selected adjunctive therapy, the patient can have considerable control over the disease. It has been found that there are patients with experience in alternative methods of treating bronchial asthma. During interviews with the nurses were apparent, that their role in the allergology clinic is irreplaceable and fulfills an important function, without which doctors would find it difficult to do. For practice, I would recommend to adolescents and their parents education about the issue of the disease, the symptoms of the disease and treatment options.
Fenotype - Based Treatment of Severe Asthma Bronchiale in the Czech Republic

INTRODUCTION In recent years there has been a lot of evidence that asthma is etiopathogenetically widely heterogeneous inflammatory disease. Simultaneously with this knowledge, there are new therapeutic possibilities of severe asthma, ie. targeted biological therapy, which reflects their inflammatory heterogeneity. A prerequisite of rational indications of this treatment requires closer asthma phenotyping using available biomarkers. AIM The aim of submitted work is as follows: A) Overview of diagnostics and treatment of asthma in the world and in the Czech Republic with focus on monitoring the effect of targeted therapy of severe asthma selected on the basis of phenotyping using biomarkers. B) The work is divided into three parts. 1. In the first part it is about evaluating the effect of treatment with anti IgE antibody omalizumab in the Czech Republic since 2007. 2. In the second part, the relationship between the specific allergic profile of patients to the results of treatment with omalizumab in the Czech Republic is examined in detail. 3. Karolína Švrčková Title: Physiotherapy of children with lung disease Objectives: The aim of this thesis is to create a targeted therapeutic program for school- age children diagnosed with bronchial asthma and to evaluate the change of the examined parameters (FEV1, dyspnoea, muscle imbalance, resting breathing wave, diaphragm test, five-minute running test with a focus on post-exercise dyspnoea, viscerovertebral manifestations) after four weeks of movement in two groups (one focused on respiratory physiotherapy, the other on respiratory physiotherapy enriched with yoga exercise), both diagnosed with bronchial asthma. Methods: This work is a pilot experimental study. Two groups of probands (test and control, each with eight probands) were created. The test group underwent respiratory physiotherapy and yoga exercise specifically designed for diagnosis bronchial asthma. The control group underwent only respiratory physiotherapy. The matters of patients foreknowledge about inhalant systems.

This bachelor thesis follows up the problems of awareness of patients about inhalation systems. It focuses primarily on who and how the patients are informed about the handeling with inhalation systems, how patients can handle with inhalation systems, which mistakes they are doing, and which inhalation system suits the patients best. It is about patients, who have bronchial asthma of they suffer from chronical obstructive pulmonary disease. The theoretical part deals with bronchial asthma and chronic obstructive pulmonary disease. It focuses on definitions of both asthma and chronic obstructive pulmonary disease. It also deals with their causes, symptoms, classification, diagnosis and treatment. In the next part, this bachelor thesis deals with are inhalation systems, types of inhalation systems, and their handling. At the end of the theoretical part is mentioned the role of the nurse in the education proces of the patient, within the proper handling of the inhalation system. The research part was processed by qualitative research. The data collection technique was semi-structured interview and observation. The interview was made up of 21 questions and the observation was written down in an observation sheet. The research group consisted of 11 patients, 4 of whom had chronical obstructive pulmonary disease and 7 patients had bronchial asthma. It came out from the research that the informations are forwarded to the patients by the doctors. Nurses are not very involved in forwarding informations to the patients, even though they have the necessary competencies to do so. Furthermore, the research had revealed that nurses spend only a few time with patients, in some cases just a few minutes. As part of a doctor's check, it became clear that nurses only control the patient's inhalation technique, only when patients are learning to handle with the inhalation system. Over time, they do not control the inhalation technique of the patients and therefore patients practice their inhalation technique wrong and make mistakes. It has emerged that the most widely used inhalation system is an aerosol dispenser, despite the fact that the powder inhaler is easier to handle. Furthermore,it became clear that patients were the most mistaken in aerosol dispensers. From the research came out, that the patients make the most mistakes, while using aerosol dispensers. The most common mistake are not holding the breath and not shaking the inhaler. With powder inhalers, patients made mistakes too. The fault was mainly in the exhalation through the inhaler, in the small force of the inhalation, and holding the inhalator in wrong position. Based on these results, there was made an helpful information brochure which will help all patients, who can not handle their inhalation system correctly.
Bronchial asthma in the practice of paramedic

Astma bronchiale is a lifelong chronic respiratory illness which affects all the age categories of population. The aim of this final work is to obtain knowledge and to analyse paramedics´ approaches used during a diagnosis and providing prehospital urgent care to patients affected by this illness.The bachelor´s work is divided into a theoretical and a research part. The theoretical part deals with the anatomy of the respiratory system, process of breathing, diagnosis and treatment in prehospital urgent care.The research of this work was realized by using recorded interview with paramedics. The aim of this research is to show the reader more viewpoints to this issue and to outline the experience of paramedics with treatment of this illness. This research demostrates that paramedics get in contact with the asthma bronchiale several times in a month, they are familiar with this illness and they know methods of its treatment. The result of the work shows how difficult the diagnostics of this illness is. Differential diagnostics also points to broad-spectrum of illnesses. Eventually, we can also enroll a wrong feedback between emergency service and medical facility. Results of this work in the theoretical part help the reader to get objective information about this matters, diagnostics and treatment of this illness in prehospital urgent care. The main contribution of this work should also be the view to the methods, approaches and experiences of paramedics during the treatment of the asthma bronchiale in prehospital urgent care. The output of the bachelor´s work is a brief manual for paramedics showing how to proceed during diagnostics and treatment of the asthma bronchiale.
The specifics of nursing care of a child with asthma bronchiale

Current state: Asthma is a disease, which is on the rise in terms of spread. The scientific investigations is focused on nursing care of children with asthma bronchiale and other important areas such as diagnosis, treatment, complications or prevention. Amongst basic symptoms are coughing, stuffiness, and sound phenomena such as wheezing and rustling. The symptoms can escalate to a strong asthmatic attack, which is life-threating to the person suffering from it. The objective was to evaluate the knowledge of nurses in nursing care of a child with asthma bronchiale. Another objective of the research study was to evaluate inform parents of children with asthma bronchiale about the disease. Methodology: The chosen method of scientific investigation is the method of qualitative research. The method of collection of data is an unstructured in-depth interview with nurses and parents of children with asthma bronchiale. All interviews were, after approval of all communication partners, audio recorded and afterwards they were rewritten and analyzed using the method of open coding using the technique "pen and paper". The scientific sample is made of nurses and parents of children with diagnosed asthma bronchiale. The first scientific sample are four nurses working at the children's department of hospitals, two nurses working at ambulances of general practitioners for children and youth and two nurses working at the ambulances of outpatient specialists. The second scientific sample is made of seven parents of children with diagnosed asthma bronchiale. The choice of scientific subject is intentional. The size of scientific samples is set by the saturation of data acquisition. The results of the investigation show, that nurses have corresponding knowledge connected with crucial specifics of the nursing care of children with asthma bronchiale. The nurses are able to describe all the differences in the nursing care of the child according to its age group. Knowledges of nurses about the disease itself are extensive. They are also well-versed in terms of education. They also introduced in which way parents of sick children are involved in the nursing care. Involvement in nursing care was confirmed by the parents themselves. Parents' knowledge of the medical and nursing care of a child with bronchial asthma was sufficient. All of questioned parents have wide knowledge, even parents of children with very recently diagnosed asthma. Conclusion: Asthma bronchiale is a very common illness, and its spread in population is rising. This thesis can be used by nurses, other medical staff and parents to obtain more information about this disease, its treatment, complications, nursing care, educations and prevention. At the end of the thesis are summarized the main nursing activities of the children with asthma bronchiale. Based on the results of the study, the main principles of childcare with bronchial asthma in the home environment were proposed.
